Multiple Procedure (51) & Bilateral Surgery (50) Payment Estimator
Rank same-session procedures to see standard 100%/50% multiple-procedure pricing, or price a bilateral service by its MPFS bilateral indicator - 150%, no adjustment, or already bilateral-priced.
Standard Multiple-Procedure and Bilateral Rules (CMS MPFSDB Indicators)
From CMS Transmittal R1777CP and SE1422 (Jan 2018), with Noridian JE's MPFS indicator descriptors: under standard multiple-procedure rules the highest-valued procedure is paid at 100% and each subsequent procedure at 50%. For bilateral services the indicator decides everything: 1 = payment at 150% of the single-code amount (one line, modifier 50, one unit); 0 = no 150% adjustment (100% basis, lower of charges); 2 = the fee schedule amount is already bilateral-priced; 3 = priced by report. Endoscopy families and add-on codes follow different multiple-procedure rules and are excluded from the multiple mode.
KX modifier threshold: have you crossed $2,480 this year?
For CY 2026 the KX threshold is $2,480 for PT and SLP combined and $2,480 separately for OT (CMS Therapy Services CY 2026 update; re-confirmed at each November re-check). Crossing it is not a cap and not permission: append KX only on claims where continued therapy is medically necessary and the record documents it. The year-to-date fields in the allocator above run this check on every calculation.
Frequently Asked Billing Questions
Do I append modifier 51 myself or does the payer add the reduction?
Append modifier 51 to the secondary procedure lines where the code's MPFS multiple-procedure indicator calls for standard rules; payers then apply the 100%/50% pricing. Never append it to add-on codes or to the primary (highest-valued) line.
