Medicare Part B JW & JZ Drug Waste Billing Unit Calculator

Convert administered mg/units and single-dose vial sizes into exact CMS-1500 / 837P claim lines—including automatic protection against the sub-unit discard trap and multi-dose vial rejections.

Worked example (CMS JW/JZ FAQ case): 350 mg infliximab from four 100 mg single-dose vials at 10 mg per unit = Line 1: J1745 ×35 units (administered) + Line 2: J1745-JW ×5 units (discarded). Now enter your own dose below.

Verified CY 2026 CMS Single-Dose Drug Wastage Rules (JW vs. JZ)

Dose to billing units (before waste): the conversion every JW/JZ claim starts with

Divide the administered dose by the HCPCS descriptor unit (for example 350 mg ÷ 10 mg per unit = 35 units) before deciding anything about waste. Units come from the code descriptor, never from vial size. Only then open enough single-dose vials, subtract, and let the waste rule below pick JW, JZ, or neither.

Under CMS Change Request 13056 (MLN Matters MM13056), every Medicare Part B claim for a separately payable drug from a single-dose container must carry either -JW (when ≥1 full billing unit is discarded) or -JZ (attesting zero billable units discarded).

KX modifier threshold: have you crossed $2,480 this year?

For CY 2026 the KX threshold is $2,480 for PT and SLP combined and $2,480 separately for OT (CMS Therapy Services CY 2026 update; re-confirmed at each November re-check). Crossing it is not a cap and not permission: append KX only on claims where continued therapy is medically necessary and the record documents it. The year-to-date fields in the allocator above run this check on every calculation.

Frequently Asked Billing Questions

When is the JZ modifier mandatory instead of JW?
Use JZ on a single claim line whenever the full single-dose vial is administered OR when the discarded remainder is smaller than one full HCPCS billing unit.

Can JW or JZ be used on multi-dose vials?
No. CMS Medicare Claims Processing Manual Chapter 17 Section 40 strictly limits JW and JZ to single-dose containers. Multi-dose vial waste is never separately payable.
