Infusion Administration Time & Units Worksheet
One initial service per encounter, additional hours only past the 30-minute mark, sequential drugs once each, concurrent once. Enter the primary drug's documented minutes and let the worksheet apply the time rules.
The Infusion Hierarchy and Time Rules This Worksheet Applies
Per AAPC Knowledge Center drug-administration guidance (Dec 2023) and the University of Wisconsin IV Infusion Mini Manual (Sep 2024), stated in our own wording: only one initial service is reported per encounter, chosen by hierarchy (chemotherapy first, then therapeutic, then hydration). An infusion must exceed 15 minutes to use an infusion code at all; each additional hour requires more than 30 minutes beyond the prior hour (so 61–90 minutes is still one unit of time); a sequential infusion is a different drug given after the primary through the same access, once per drug; a concurrent infusion runs at the same time and is reported once per encounter.
| Primary minutes | Initial | Additional hours |
|---|---|---|
| 1–15 | Not an infusion — evaluate IV push | |
| 16–90 | ×1 | 0 |
| 91–150 | ×1 | 1 |
| 151–210 | ×1 | 2 |
KX modifier threshold: have you crossed $2,480 this year?
For CY 2026 the KX threshold is $2,480 for PT and SLP combined and $2,480 separately for OT (CMS Therapy Services CY 2026 update; re-confirmed at each November re-check). Crossing it is not a cap and not permission: append KX only on claims where continued therapy is medically necessary and the record documents it. The year-to-date fields in the allocator above run this check on every calculation.
Frequently Asked Billing Questions
Why does a 75-minute infusion earn no additional-hour unit?
Additional hours are earned only when the infusion runs more than 30 minutes past the prior hour mark. At 75 minutes you are 15 minutes into the second hour, so only the initial hour is reported; the second additional-hour unit begins after 90 minutes.
